Position Summary:

The Case Manager Supervisor in the Permanent Supportive Housing (PSH) Program provides leadership and oversight to ensure the effective and compliant delivery of participant-centered supportive services to individuals and families who have experienced chronic homelessness and are medically vulnerable with significant barriers to housing stability.
The Supervisor oversees a team of case managers and supports the implementation of services consistent with HUD Continuum of Care (CoC) requirements, Housing First principles, Coordinated Entry policies, and applicable program and organizational standards. The position is responsible for ensuring that participants receive individualized, and strengths-based services designed to promote housing stability, health and wellness, community integration, and increased self-sufficiency.

Duties and Responsibilities:

Supervision:

  • Supervise, train, mentor, and support a team of case managers.
  • Conduct monthly supervisions with the case managers while providing guidance and direction to program staff, ensuring they adhere to program guidelines and best practices.
  • Conduct regular team meetings for communication of program updates and case conferencing to enhance staff performance.
  • Monitor service delivery to ensure that performance measures and contract requirements are met.
  • Conduct case management internal audits of case notes and case manager documentation.

Program Management:
  • Oversee the day-to-day operations of the PSH Program.
  • Monitors client progress, ensuring the implementation of effective case management strategies through monthly By Name list meeting.
  • Assists case managers in conducting assessments and developing individualized service plans for clients.
  • Coordinates services using a wraparound/HOST/team approach.
  • Creates and maintains consistent communication channels, both verbal and written, between several parties (i.e. tenant, landlord, referral source, collaborating agencies).
  • Collaborate with other supervisors and program managers to enhance efficiency.
  • Serves as an ongoing liaison between property managers and participants as well as between participants and neighbors.

Client Support and Engagement:
  • Provides case management conducting home visits through for individuals and family in our Permanent Supportive Housing program who are chronically homeless and medically vulnerable as identified through the Continuum of Care.
  • Travels and conduct home visits and complete home inspections to meet participants, while being flexible to work an adjusted work schedule including evenings and weekends as necessary.
  • Refers clients to essential services such as healthcare, mental health counseling and job training.
  • Addresses crisis situations promptly and effectively, providing necessary support to clients.
  • Provides mediation and advocacy with landlords on the participant’s behalf to develop a workable plan to maintain housing.
  • Provides transportation to medical services sites and dental services as well as information on other resources the individual may access to improve their health and/or level of self-sufficiency.
  • Documents all services identified and provided, referrals made, and transportation provided.

Data Management and Reporting:

  • Maintains accurate and up-to-date client records in compliance with program standards and regulations into HMIS.
  • Prepares regular reports detailing program outcomes, challenges, and successes.
  • Collaborates with the data management team to ensure accurate data entry reporting.

Community Collaboration:

  • Establishes and maintain positive relationships with community partners, service providers, and housing agencies.
  • Attends meetings, conferences, and workshops to stay updated on best practices and industry trends.
  • Advocates for clients’ needs within the community, fostering a supportive network for program participants.

Required Skills/Abilities:

  • Bachelor’s or master’s degree in social work, Counseling, Psychology, or related field.
  • Minimum of 3 years of experience in case management, with 2 years in a supervisory or leadership role.
  • Experience in mental health strongly preferred.
  • Strong understanding of homelessness, housing programs, and social services.
  • Excellent communication, leadership, and organizational skills.
  • Ability to work collaboratively within a team and across various community organizations.
  • Proficiency in data management software (HMIS) and Microsoft Office Suite,including Microsoft Excel.
  • Commitment to social justice and empowering individuals experiencing homelessness.
  • Ability to work flexible schedule and manage workload sufficiently to meet deadlines.
  • Ability to lift items weighing 10-20 pounds.
  • Valid driver’s license, reliable transportation, good driving record, and personal car insurance.
  • Bilingual (English/Spanish) a plus.
  • Ability to drive a 15-passenger van.
  • Must pass criminal background and Child Protective Services background checks.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times. 
  • Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year.

Salary Range:

$58,000 - $65,000 annually.
